GP Care Plans for Physiotherapy

What is a GP Care Plan?

A GP Care Plan - also known as a Chronic Disease Management Plan (CDMP) or EPC Plan - is a Medicare initiative that helps people with long-term or chronic health conditions access allied health services like Physiotherapy and Dietetics.

If your GP decides you’re eligible, they can refer you for up to 5 allied health visits per calendar year. These visits usually attract a Medicare rebate of $63.40 per session (if you have reached the Medicare Safety Net you may receive more back).

Am I eligible?

You may be eligible for a GP Care Plan if you have a health condition that has lasted, or is expected to last, for 6 months or longer. This might include conditions such as:

  • Chronic Back or Neck Pain

  • Osteoarthritis

  • Pelvic Health, Incontinence or Prolapse

  • Pelvic Pain or Pelvic Girdle Pain in Pregnancy/Post partum

  • Ongoing muscle or joint pain

  • Balance or mobility issues

Your GP will assess your eligibility and create a plan tailored to your health needs.

How it works at FitRight

Firstly, you will need to visit your GP and ask if you are eligible for a Care Plan referral to see a Physiotherapist or Dietitian at FitRight.

Your GP will either send us your referral directly (we will contact you via text/email once received) or will they give you a paper copy of the plan. If they give it to you, please email us a copy at hello@fitright.au prior to your appointment.

You are welcome to book your appointment online or contact us on (08) 6444 7994 between 8.30-3pm Monday to Friday if you are unsure which appointment to book.

When your appointment is complete you will need to pay the full consultation fee on the day. We will process your Medicare rebate (usually $63.40) on the spot and it will usually appear in your bank account within 24-48 hours.

Please note: we are a private billing clinic, so out-of-pocket expenses apply.

Frequently Asked Questions

Do I need to bring anything to my first appointment?

Please send your GP Care Plan referral to hello@fitright.au prior to your appointment.

Please bring your Medicare card and any relevant scans or reports (e.g. ultrasound, MRI, X-rays) to your appointment.

Can I use private health insurance as well?

You can’t claim both private health insurance and Medicare for the same session. You’ll need to choose one or the other for each appointment.

Can I use my 5 visits across multiple allied health providers?

Yes - the 5 visits are shared across all allied health professionals listed on your Care Plan (e.g., physio, dietitian, podiatrist).

While we have record of your appointments, it is your responsibility to keep track of your 5 appointments to ensure that you receive your expected rebate.

Can I Use My Care Plan for Classes?

You can use your GP Care plan visits for Clinical Exercise at FitRight. You will need to have an initial assessment first (which you can use one of your visits for) before joining our small group classes. These classes are individualised for each participant.

GP Care Plan visits cannot be used for our Group Classes, including Baby&Me, Bump&Me, Empower and New Mums Aqua.

Can I Use My Care Plan for Telehealth appointments?

Yes - Medicare rebates can be claimed for eligible Telehealth Physiotherapy and Dietitian sessions, provided your GP has included Telehealth in your referral.
